Knee osteoarthritis is one of the most common causes of chronic joint pain, and physical therapy is a standard first-line treatment for it. A large randomized trial asked whether Tai Chi — a slow, low-impact mind-body exercise — could work just as well. The results, published in a leading medical journal, suggest that it can.

What the research looked at

Researchers ran a 52-week, single-blind randomized controlled trial published in the Annals of Internal Medicine. The study enrolled 204 people with knee osteoarthritis, with an average age of about 60 years; roughly 70% of participants were women. Participants were randomly assigned to one of two groups: a Tai Chi program, practiced twice weekly for 12 weeks, or a standard physical therapy program, the usual comparison treatment recommended for knee osteoarthritis. Because the trial was single-blind, participants knew which treatment they were receiving, but some of the people assessing outcomes did not — a design meant to reduce bias when interpreting results.

What it found

Over the full 52 weeks of follow-up, Tai Chi performed as well as physical therapy on the two main measures researchers use to judge knee osteoarthritis treatments: WOMAC pain score (a standard scale for rating osteoarthritis pain) and physical function. In other words, people who did Tai Chi improved in pain and function about as much as people who received physical therapy — neither treatment came out clearly ahead of the other. The two treatments also had a similar safety profile, meaning Tai Chi did not appear to carry more risk of harm than standard physical therapy. As an added finding, Tai Chi also helped participants' mood, a benefit not typically expected from physical therapy alone.

What it means (and the limits)

The study authors concluded that standardized Tai Chi should be considered an effective therapeutic option for people with knee osteoarthritis, alongside or as an alternative to standard physical therapy. That is meaningful because Tai Chi is low-cost and low-impact, and once learned it can be practiced in groups or at home, which may make it easier for some people to keep up long term than a structured physical therapy program. That said, this is a single trial, and like all research it has limits: it compared Tai Chi with one specific physical therapy protocol, in one population of adults with knee osteoarthritis, and a single-blind design cannot fully rule out the influence of participants' own expectations on how they rated their pain. It does not establish that Tai Chi helps every joint condition, and it says nothing about supplements, medications, or other interventions. Anyone considering Tai Chi for knee osteoarthritis should talk with their doctor or physical therapist about whether it is a good fit for their specific joints and overall health.
