Bone and cartilage sit right next to each other inside every joint, and it is easy to think of them as more or less the same kind of tissue. Structure: Rigid Scaffold vs Smooth Cushion
Bone is a rigid, mineralized tissue built from a collagen framework hardened with calcium and phosphate crystals. This combination gives bone both strength and a degree of flexibility, allowing it to bear weight and resist fracture under normal loads. Bone is also a living, dynamic tissue: it contains blood vessels, nerves, and specialized cells that constantly remodel it throughout life. Cartilage, by contrast, is a smoother and softer connective tissue made mainly of collagen and water-attracting molecules called proteoglycans, with cartilage cells (chondrocytes) scattered sparsely throughout. At the ends of bones inside a joint, a thin layer called articular cartilage provides a low-friction, shock-absorbing surface that lets bones glide over each other smoothly during movement.
Blood Supply: A Key Difference
One of the most important distinctions between bone and cartilage is blood supply. Bone is richly vascularized, with a network of blood vessels running through it that deliver oxygen, nutrients, and the cells needed for repair. Articular cartilage, on the other hand, generally has little to no direct blood supply of its own in adults. It relies instead on the surrounding synovial (joint) fluid to slowly diffuse nutrients in and waste products out. Cartilage also has no nerve endings, which is part of why cartilage damage itself is often painless in its early stages — pain tends to arise later, once nearby bone, the joint lining, or surrounding tissues become involved.
Healing: Why Bone Mends and Cartilage Struggles
This difference in blood supply largely explains why bone and cartilage heal so differently:
- Bone has an active healing response. After a fracture, blood vessels deliver the cells and building blocks needed to form new bone tissue, and with proper immobilization and time, a break generally mends and can become as strong as before.
- Cartilage has a much more limited capacity for self-repair. Because it lacks a direct blood supply and has very few cells, damaged cartilage does not regenerate the way bone does. Minor surface wear may sometimes be managed by the body, but significant cartilage loss generally does not grow back on its own.
Relevance to Osteoarthritis
Osteoarthritis (OA) is, at its core, a story about cartilage's limited healing ability. Over time, the cartilage cushioning a joint can thin and roughen faster than the body can maintain it, reducing its shock-absorbing capacity. As cartilage wears down, the underlying bone can also change — including thickening at the joint margins (bone spurs) and increased stress on the bone directly beneath the remaining cartilage. This is why OA is often described as affecting the "whole joint," not cartilage alone, and why it tends to be a gradual, progressive condition rather than something that reverses quickly.
